Darchula, one of the mountainous districts of the Sudurpashchim Pradesh of Nepal is situated at an altitude ranging from 518 to 7132 m above sea level and covers 2322 square km of Karnali Province. It also lies between 290, 36' and 300,15' northern latitude and 800,22' and 810,09' eastern longitude on the world map. The district is surrounded by Baitadi from the south, Bajhang from the east, Uttarakhand Pradesh of India from the west, and Tibet of china from the north.

Darchula district is politically divided into 2 Municipality and 7 Ruralality, house of representative constituency 1 and provincial constituency 2. It's District Headquarters (DHQ) is Khalanga, located in Mahakali Municipality on the bank of Mahakali river connected with Dharchula city of Uttarakhand, India. The Gokule-Darchula highway stretches 71.78 km between Baitadi and Khalanga.

Khalanga, Gokuleshwore, and Latinath bazaar are the three main trade center of this district. An Airport located at Gokuleshwore, about 71 km from DHQ, was not functioning since conflict and its reconstruction has not yet begun. The major rivers that flow through this district are Mahakali, Chaulani, Kalagad, Tinkarkhola, Naugad, Laskukhola, Hoparigad, Makarigad, etc. The Malikarjun temple, Latinath temple, Api Himal, Nampa Himal, Byas Himal are the most popular tourist site in the district. The total population of Darchula is 1,33,274 of which 47.72% are male and 52.28% are female.

The sex ratio of this district is 91.30 males as per 100 females and 57 people per square kilometer are the population density of this district. Only 0.88% is the population growth rate of this district. This seasonal migration mainly takes place generally from October to March (agricultural off-season), when there are limited job opportunities. Most of the migrants come from ultra-poor, socially excluded, and marginalized groups, such as the Dalits. Internal migration is also common, with some people moving to the plains or terai for work. 

Objectives:

Constitution of Nepal - 2072 aims to ensure basic health as the fundamental right of people. Thus, health is the basic right of people by law. Primary health care services will be provided according to the proclamation of the Alma Ata Declaration. Thus, DHO has mainly the following objectives to serve people according to the spirit of the government and the people: To bring the preventive, curative, and promotional health services up to the doorsteps of people. Monitoring and supervision of health services.  

Tasks: ·        

  • To determine the requirement of manpower for health institutions in the district. ·        
  • To ensure the supply of drugs, equipment, instruments, and other materials at different health institutions in the district. ·        
  • To ensure effective implementation of public health programs in the district. ·        
  • To manage the immediate solution of problems arising from natural disasters and epidemics in the district at different levels.      
  • To foster coordination with external development partners for effective delivery of resources and health services in the district.     
  • To systematically maintain data, statements, and information regarding health services in the district, update and publish them as required. 
  • To identify gaps in health service delivery in the district and seek a solution.       
  • Supervision/Monitoring in all health institutions in the district.  

Roles and Responsibilities of District Health Office::

District Health office was established in the year 2028 BS (1971 AD) on the basis of the concept of decentralization policy. The office is located in Darchula. Its existence continued even after the re-establishment of DoHS in 2051 BS. DHO, Darchula is functioning under the FWRHD and DoHS. It has a direct strong functional relationship with FWRHD, and with other health-related organizations of the district.
